As a Patient-Centered Medical Home (PCMH), our patients receive high quality evidence-based primary care from a team of nurse practitioners. Our care team is committed to partnering with our patients, their families, and caregivers so that all patients get the best care they need. That means you can expect members of our care team to:

  • Contact you to remind you about upcoming appointments and tests
  • Ask you to identify who you would like to be your primary provider and will assist you in choosing one, should you need it
  • Provide evidence-based resources and self‐management tools to help you manage your care on your own

You are the most important member of your care team. We encourage you to ask any questions you may have about your health and how we can support you.
