Your Medical Home
As a Patient-Centered Medical Home (PCMH), our patients receive high quality evidence-based primary care from a team of nurse practitioners. Our care team is committed to partnering with our patients, their families, and caregivers so that all patients get the best care they need.
What to Expect from our PCMH Care Team
You should expect members of your care team to contact you to remind you about upcoming appointments and tests. Members of the care team will ask you to identify who you would like to be your primary provider and will assist you in choosing one, should you need it. The care team will also provide evidence-based resources and self‐management tools to help you manage your care on your own. As an active member of the care team, you are encouraged to ask members of your care team any questions about your health. View a selection of patient resources.

What to Bring to On Your Visit
We are committed to managing all aspects of care, and to do that we need the following reports and information. If you do not have these items,
members of our team will work with you to obtain them.

Medical Information
To provide our practitioners with a complete background of your medical history, we request access to your last annual physical, immunization records, recent lab and test results, a transition plan for adolescent and young adult patients, the names of other providers you have seen or are seeing currently, and records from recent hospitalizations or emergency room visits.

Medications and Pharmacy Info
We need a list of all medications you are taking and the current dosage, as well as any vitamins and/or supplements. We also need the contact information of the pharmacy you use.

Medicaid, Medicare or Private Insurance Card
Please bring your insurance card on all visits. View a list of accepted health plans.

Coordinating Your Care
Referrals
The Nurse Practitioner Group care team manages all aspects of a patient’s care, including referrals. If you require a referral, our nurse practitioners will work with you to find the right specialist from our world-class network of specialists from Columbia University Irving Medical Center. Our care team will stay involved to coordinate your care as your primary care providers.
Hospitalizations, Emergency Room and Urgent Care Visits
If you feel that you need emergency care (in non‐life threatening situations), please first contact your care team at the Nurse Practitioner Group at 212-326-5705
If you do end up going to the hospital, please help us coordinate your care by alerting us that you had a hospitalization, emergency room, or urgent care visit. Following your visit, we will attain your discharge information from the providers who cared for you and schedule a follow‐up appointment with you at the Nurse Practitioner Group to ensure your primary care needs are being met.
Mental Health
To ensure your mental health needs are met, you can access mental health services at the Nurse Practitioner Group in Washington Heights. Our mental health provider will communicate your needs with your other health providers so that care is properly coordinated.
Accessing Care
The Nurse Practitioner Group offers same-day appointments based on availability.
*If your appointment is following a hospital or ER visit, please inform the Registration Associate.
After Hours Care
For medical questions after hours please call 212-326-5705 to reach our after hours answering service. If your issue requires urgent medical attention, the answering service will contact the on-call provider to get in touch. If your issue is not urgent, you will be directed to leave a message to be received by the Nurse Practitioner Group during the next business morning.
